Start with the document you need

Use Connect for documents already in your account. For a missing hospital note, office record or scan, first identify the team that holds it. The KeepMD route covered here is Columbia University Irving Medical Center; it does not establish coverage for every NYP affiliate.

Which NewYork-Presbyterian / Columbia records can you bring together?

Use these ten categories to check your connected records. What arrives depends on what the hospital has recorded and shares for your account and dates of care.

10 categories to check

10 types

Use this checklist for an Epic connection. Available content depends on your account; listing a category does not confirm that records in it have been imported.

  • Lab results
Blood work and other laboratory results. Check the collection date, units, and reference range when comparing tests from different visits.
  • Vital signs
Measurements such as blood pressure, pulse, height, and weight. Look at the visit date so an older measurement is not mistaken for a current one.
  • Medications (prescriptions)
Prescription records may include current and past orders. A prescription entry does not establish what you take today; discuss discrepancies with your care team.
  • Allergies and intolerances
Recorded allergies, intolerances, and reactions. An empty list does not confirm that you have no allergies; compare it with information you have given your clinician.
  • Immunizations
Vaccinations documented in the chart. Doses given at a pharmacy, school, or another health system may need to be obtained from that source.
  • Health conditions and problems
Diagnoses and entries on the problem list. Some entries describe past care; ask the treating team about an unfamiliar or incorrect entry.
  • Procedures
Documented procedures and treatment history. For the details of an operation, look for the operative report as well as the procedure entry.
  • Visits and encounters
Office visits, emergency visits, and hospital stays. Compare the facility and dates with your own appointment or discharge paperwork.
  • Clinical documents and notes
Released clinical notes, visit summaries, and other documents. Availability can differ by document and date; request a missing note by visit and clinician.
  • Diagnostic reports
Written findings from imaging or other diagnostic tests. The report explains the findings; the actual scan images may follow a separate download or request process.

Not yet verified

1 type

These may exist in the portal, but KeepMD has not verified their availability through this connection.

  • Categories available in your account
Connect coverage depends on the patient, encounter and permissions. Compare each category with the portal; this guide does not certify every category as imported.

Usually a separate request

2 types

Records NewYork-Presbyterian / Columbia typically releases through its medical-records process, not through the patient-access connection.

  • Images and reports
The report and the image files are different records. Confirm the imaging location and study date before requesting a copy.
  • Complete chart or certified documents
Use the formal request route for the correct custodian; a portal view can be partial.

Connect is NewYork-Presbyterian’s patient portal. The KeepMD connection covered here is listed under Columbia University Irving Medical Center. Start with your Columbia visit and confirm the patient profile before sharing access. If you also received care at another NYP hospital, check that visit separately. A familiar system name does not establish that every campus sends records through the same connection.

Review and download your documents

For records already available in Connect, open the menu, find My Record and choose Document Center. NYP also offers a request inside Connect for documents that are missing. That route can return the requested material to your Document Center. Before a specialist visit, make a checklist of the admission dates, discharge summary, relevant test reports and any document the receiving office specifically asked for. Compare the downloaded packet with that checklist instead of treating a successful download as proof that the chart is complete.

Request the right records

The NYP records page provides separate choices for patients or representatives and for attorneys. Choose the patient route for your own records. Paper authorizations are also available in English, Spanish and Chinese. If a relative is helping, establish their authority through NYP’s process rather than sharing a password. Records-authorization help is 646-697-4764; Connect account help is 646-962-4200. Keep these two purposes separate when calling so the question reaches the right team.

Review access before authorizing

Authorize through NewYork-Presbyterian / Columbia's official portal and confirm the patient. Follow the hospital's proxy permissions when helping a relative.

Keep passwords and sign-in codes private. To stop future app access, check the portal's connected-app controls. Removing permission does not automatically delete files already downloaded or shared. KeepMD is independent of NewYork-Presbyterian / Columbia and Epic. The screens are examples with synthetic values, not evidence of a hospital-specific import.

Review imported records and permissions

After connecting, compare a known Columbia encounter with the official portal. Check the date, source location and document type before deciding something is missing. If the visit is absent from Connect, contact its records custodian. If it is visible there but missing from KeepMD, report the facility, approximate date and missing category without sending passwords or the entire chart. Corrections to the original clinical record belong with the team that created it. Reimporting does not amend that source record.

Known NewYork-Presbyterian / Columbia quirks worth checking

The visit is missing

Hospital records and physician-office records can have different custodians. NYP’s contact directory separates hospital requests from ColumbiaDoctors and Weill Cornell Medicine outpatient requests. Write the treating location and clinician on your checklist. For example, a Columbia hospital discharge and a later office appointment may need two follow-ups. Use the official directory to choose the destination for each item, and retain the request confirmation. Do not send a full chart to an unfamiliar email address found in a search result.

You need original images

A radiology report describes a scan, while the image files let another radiology team review the examination itself. NYP provides location-specific radiology contacts. ColumbiaDoctors also publishes radiology records guidance for its own imaging locations. Find the place where the examination occurred and ask that office for the study date and delivery format your next clinician needs. A portal report or a KeepMD record category should not be taken as a promise that the original images are included.
